Admission application and other forms provide student name, address, age, birth date, race, nationality, marital status, number of children, religion, and phsyical condition; emergency contact name and address; schools attended; degrees earned; courses taken; hours spent; classwork and practical grades.

The Illinois General Assembly instructed the Board of State Commissioners of Public Charities to plan the Illinois State Colony for Epileptics (1899) but construction and operational funds were not appropriated until 1913.
